Reporting Immunization Status to the Health Department

Michigan schools and licensed childcare centers are required to report Immunization Program (IP) compliance on students through the age of 18 to their local health department.

Assistance with changing Site Administrators, forms, and MCIR registration or access question, contact the MCIR Help Desk. Reporting questions including help with waivers should be sent to the Local Health Department.
